A :   I wish there was a quick and simple answer, but this is NO quick and simple situation...ever.  

Ideally, it is when the Senior is starting to notice a decline in their ability to meet their own activities of daily living (ADL's).  It's great to be pro-active and have a plan in place, however most Care Managers are called in once there is an emergency hospitalization or crisis.
